All new clients must fill out an intake form for Body Thyme Massage. If this is your first time coming to Body Thyme, or if your information has changed, please download and print the form/s below to bring with you to your session. If you choose to complete the intake form on site prior to your massage, please arrive 10-15 minutes early.
